Infant Mortality Death Rates take a huge toll on World Life Expectancy rankings. It's true most of the problems are in Developing countries such as Africa, but there can be dramatic differences in places like India and China. For example the death rate is 39.6 per 1000 live births in India and only 11.2 per 1000 live births in China, which has greatly improved. In the Developed world there are dramatic differences as well. The United States death rate is 5.2 and Japan is 1.9 per 1000 live births.
